Worked out from Pugh RNH, Murray-Lyon IM, Dawson JL, Pietroni MC, Williams R. Transection of the oesophagus for bleeding oesophageal varices and 2 other sources.
Ascites and encephalopathy are graded by clinical judgement, so two clinicians can score the same patient differently. This is the known weakness of the index and the reason MELD replaced it for organ allocation.
The survival figures attached to each class are the conventionally quoted historical estimates, not a contemporary cohort.
A severity score, not a prognosis for an individual. Two of the five items are subjective clinical gradings, and the survival percentages come from historical cohorts that predate current treatment. Interpretation belongs to a hepatology team.
How this is worked out
5 steps
- Bilirubin 2.50 mg/dL against the standard bands < 2 / 2-3 / > 3 mg/dL: 2 point(s).
- Albumin 3.10 g/dL: 2 point(s).
- INR 1.8: 2 point(s).
- Ascites (mild): 2 point(s). Encephalopathy (none): 1 point(s).
- Total 9 of a possible 15, which is Class B.
Pointstable
| item | value | points |
|---|---|---|
| Total bilirubin (< 2 / 2-3 / > 3 mg/dL) | 2.50 mg/dL | 2 |
| Serum albumin (> 3.5 / 2.8-3.5 / < 2.8 g/dL) | 3.10 g/dL | 2 |
| INR (< 1.7 / 1.7-2.3 / > 2.3) | 1.8 | 2 |
| Ascites | mild | 2 |
| Hepatic encephalopathy | none | 1 |
