Absolute Count Calculator

Absolute neutrophil count from the WBC and the segs and bands percentages (or from absolute differential counts), graded by NCI CTCAE v5.0 and the mild/moderate/severe clinical scale.

Absolute neutrophil count
3,480 cells/µL
Absolute Neutrophil Count (ANC) Calculator
Absolute neutrophil count
3.480 x10^9/L
CTCAE v5.0 grade (neutrophil count decreased)
Not graded (CTCAE v5.0)
CTCAE band applied
ANC >= 1500/µL (LLN 1500 leaves no Grade 1 band)
Clinical severity
Not neutropenic (>= 1500/µL)
Meets IDSA neutropenia definition (< 500/µL)
No - ANC >= 500/µL
Neutrophils used (segs + bands)
58.0 %
WBC used
6,000 cells/µL
Method
Percentage differential: ANC = WBC x (segs % + bands %) / 100
Worked out from NCI CTCAE v5.0 - Neutrophil count decreased, Grades 1-4 (NCI Thesaurus definitions served by NIH MedGen) and 8 other sources.
Boundary convention: each band includes its lower value and excludes its upper value (500 is Grade 3, 1000 is Grade 2, 1500 is Grade 1 only when the LLN is above 1500). CTCAE v6.0 (2026) thresholds are not implemented.
Educational tool. CTCAE grading and neutropenia severity are for information only; treatment decisions, including febrile neutropenia management, belong to the treating clinician.

Most reports give percentages. If the lab prints absolute segs and bands, use the absolute mode and leave the WBC out of it.
Total WBC. Typical adult range 4.5-11.0 x10^3/µL (4,500-11,000 cells/µL).
x10^3/µL and x10^9/L are the same number. Choose cells/µL only if the report shows a figure like 6,000.
If the report gives one combined "neutrophils %", enter it here and leave bands at 0 - it already includes bands.
Immature neutrophils, included in every published ANC formula. Enter 0 if not reported separately.
Used in absolute-differential mode only.
Used in absolute-differential mode only. Enter 0 if not reported.
CTCAE Grade 1 is "< LLN to 1500". Default 1500/µL (StatPearls adult range 1500-8000), which leaves no Grade 1 band; the SCNIR range starts at 1800. Use the figure printed on the report.

How this is worked out

5 steps
  1. WBC = 6 x10^3/µL (= 6 x10^9/L) x 1000 = 6000 cells/µL
  2. Neutrophil fraction = (55% segs + 3% bands) / 100 = 0.58
  3. ANC = 6000 x 0.58 = 3480 cells/µL = 3.48 x10^9/L
  4. CTCAE v5.0 (lower bound inclusive, upper exclusive): Not graded - ANC >= 1500/µL (LLN 1500 leaves no Grade 1 band)
  5. Clinical scale (StatPearls / IDSA): Not neutropenic (>= 1500/µL)

Its 5 worked examples are re-run on every release, last on 15 September 2026. What changed

What this calculator does

Absolute neutrophil count from the WBC and the segs and bands percentages (or from absolute differential counts), graded by NCI CTCAE v5.0 and the mild/moderate/severe clinical scale.

How to use Absolute Count Calculator

Enter Differential reported as, White blood cell count (WBC), WBC unit, Segmented neutrophils (segs / polys / PMNs), Band neutrophils (bands), Absolute segmented neutrophils, Absolute band neutrophils, Absolute count unit and Laboratory's lower limit of normal for neutrophils (LLN). The calculator checks the values, applies the stated method, and updates the result without saving your figures.

What the result means

The result includes Absolute neutrophil count, Absolute neutrophil count, CTCAE v5.0 grade (neutrophil count decreased), CTCAE grade number, CTCAE band applied, Clinical severity, Meets IDSA neutropenia definition (< 500/µL), Neutrophils used (segs + bands), WBC used and Method. Review the visible working and any warnings before using the answer for a decision.

Method and checks

The method cites NCI CTCAE v5.0 - Neutrophil count decreased, Grades 1-4 (NCI Thesaurus definitions served by NIH MedGen); eviQ (Cancer Institute NSW) - Haematological toxicity grading, adapted from CTCAE v5.0; Freifeld AG et al. IDSA Clinical Practice Guideline for the Use of Antimicrobial Agents in Neutropenic Patients with Cancer: 2010 Update. Clin Infect Dis 2011;52(4):e56-e93, among others. Its 5 built-in worked examples are checked on every full test run. The initial answer and working are rendered on the server, so they remain readable without JavaScript.
