ASCVD Risk Calculator

ASCVD risk calculator works out 10-year risk of a first hard ASCVD event and Guideline risk category from Risk model, Age and Sex. The default figures on this page give 10-year risk of a first hard ASCVD event of 5.4 %; change any entry and the result, the working and the sources update together.

The Pooled Cohort Equations apply to adults aged 40-79 without existing clinical ASCVD, and were derived in non-Hispanic White and African American cohorts; they are known to over- or under-estimate risk in some other populations.
The 2013 equations are the model implemented here. The AHA PREVENT equations (2023) are a newer model with a different variable set and are not computed by this engine.
Educational estimate only. The Pooled Cohort Equations are a population model for primary prevention in adults aged 40-79 without existing ASCVD; treatment decisions belong to a clinician in a risk discussion with the patient.

Updates as you type
The equations are validated for ages 40 to 79.
The model was fitted with separate African American equations; the guideline says to use the White/other equations for all other populations.

How this is worked out

4 steps
  1. Stratum: White or other men (baseline 10-year survival 0.9144, group mean 61.18)
  2. Coefficient x value sum = 60.6995
  3. Sum minus group mean = 60.6995 - 61.18 = -0.4805
  4. Risk = 1 - 0.9144^exp(-0.4805) = 5.38%

Its 5 worked examples are re-run on every release, last on 15 September 2026. What changed

What the ascvd risk calculator calculates

Estimates the 10-year risk of a first hard atherosclerotic cardiovascular disease event using the 2013 ACC/AHA Pooled Cohort Equations, with the guideline risk band.

How to use this ascvd risk calculator

Enter Risk model, Age, Sex and Race group. 10-year risk of a first hard ASCVD event updates as you type, and “Use a worked example” loads checked figures you can adjust. Change one figure at a time to see what drives the result.

How the result is worked out

Open “How this is worked out” beside the result for the step-by-step calculation behind 10-year risk of a first hard ASCVD event, Guideline risk category and Equation stratum used.

The method follows Goff DC Jr et al. 2013 ACC/AHA Guideline on the Assessment of Cardiovascular Risk. Circulation 2014;129(25 Suppl 2):S49-S73, Assessment of Cardiovascular Risk Full Work Group Report (ACC/AHA), Tables 4 and 5 - equation parameters and the worked example for each race/sex group and Grundy SM et al. 2018 AHA/ACC/Multisociety Guideline on the Management of Blood Cholesterol - 10-year risk categories (low, borderline, intermediate, high).

Guideline worked example - White man, 55, TC 213, HDL 50, untreated SBP 120 example

With Age: 55, Sex: male, Race group: white-or-other, Total cholesterol: 213, the calculator returns 10-year risk of a first hard ASCVD event: 5.384422, Coefficient x value sum: 60.6995462. This example is checked against an independent source in the test suite.

What you can compare
